____ Any necessary medications with written instructions - Heartworm medication and vitamins are administered free, there is a charge for all other medications, per administration.

Dogs: ____ WRITTEN proof of current vaccines

RABIES (Within three (3) years.)
Distemper & Parvo (Within three (3) years.)
BORDATELLA (Within one year.)
(Injectable Bordatella should not be administered within 7 days of boarding.)

We accept TITER'S for Distemper & Parvo. We also encourage a Veterinarian letter that the dog is too old, or infirm to be vaccinated.

Cats:

RABIES
FeLV (Feline Leukemis Virus)
FVRCP (Feline Viral Rhinotracheitis, Calicivirus, Panleukopnia) current within the past 12 months.
