____ Any necessary medications with written instructions (Heartworm medication and vitamins are administered free). There is a charge for all other medications, per administration.

Dogs:     ____ WRITTEN proof of current vaccines

RABIES (Within three (3) years.)
Distemper & Parvo (Within three (3) years.)
BORDATELLA (Within one year.)
(Injectable Bordatella should not be administered within 7 days of boarding.)

We accept TITER'S for Distemper & Parvo. We also encourage a Veterinarian letter that the dog is too old, or infirm to be vaccinated.

Cats:      ____ WRITTEN proof of current vaccines

RABIES(Within three (3) years.)
FVRCP (Feline Viral Rhinotracheitis, Calocivirus, Panleukopenia.) (Within three (3) years.)
FeLV (Feline Leukemia Virus.) current within the past 12 months.
